Specialty Doctors Terms and Conditions

The Framework Agreement sets out changes to the remuneration framework and terms and conditions of employment that the 2021 Specialist and Specialist contract will be adopted by the employers, BMA (NI) and the Ministry of Health (NI) during the term of the agreement and in the future. If changes are made to the General Terms and Conditions of Sale (GTC), they will be communicated to employers via the circulars on remuneration and medical and dental conditions. The change log displays all previous changes that have been made to the TCS. From April 2008, a new «specialist» was introduced, after which a contract for specialists and associated specialists (SAS) was introduced. The following list of resources refers to the 2021 Model Terms and Conditions. For more resources for the 2021 Treaty, check out the links on the main SAS Treaty reform page. We have included the general conditions for specialists and specialists. The requirement for physicians to participate in workplace planning and assessment is fundamental to the new contract, thereby broadening and developing their skill base so that they can exceed salary thresholds. The terms and conditions and guidelines for medical specialists in the 2008 contract, associate specialists and other closed SAS diplomas can be found on the old terms and conditions page. The minimum requirements to become a DS include full registration with the GMC and at least 4 years of full-time (or part-time equivalent) graduate study, of which at least 2 should be learning in a relevant field.

However, many in this class have extensive experience, postgraduate qualifications and some may even be registered in the specialist register or the general practitioner register. The term medical specialist appeared in 2008 with the beginning of the new terms of a national contract for this group of doctors who are neither trainees nor consultants. This group was previously referred to as «non-consultant career grades» and more recently became known as «SAS» physicians. Would I recommend to the new generation of physicians to at least consider this career path? Specialists are physicians and dentists who usually work in a specialty, without exception in secondary care, under the supervision of one or more consultants. It is estimated that there are probably over 20,000 doctors in this class in the UK and over 500 here in Northern Ireland. A series of changes to modernise terms and conditions to ensure they are fit for purpose as part of the evolution of the NHS to meet patient demands for care and ensure services can be provided. So the term «specialist» is new, but the rank is certainly not. SD equivalents have been around for a long time, with many different titles, but all under the umbrella of GCNC and, more recently, SAS physicians (staff ranks, associate specialists and medical specialists). There are still a few medical specialists and associate medical specialists, but these special job titles are now closed to new applicants and their number will continue to decrease as these doctors leave the workforce.

As part of the negotiations, it was agreed that the former Associate Specialist grade for new entrants would be closed as of March 31, 2008. Eligible physicians could request a personal downgrade for a period ending March 31, 2009. The terms and conditions (T&C) for SD are very similar to those for consultants, with some small differences. Like our general practitioner and consultant colleagues, SDs are required to participate in the assessment process, keep their CPD up to date and, of course, have the same responsibility to go through the revalidation process every five years. However, the biggest difference in the contract is the salary range, with the top of the SD salary scale being just below the starting point of the consultants` scale. Guide to the Appointment of Medical Specialists This document provides additional guidance to assist employers in transitioning physicians from associate specialist to specialist and in compiling evidence against the competency framework. Residency status can be obtained within 4 years of graduation and it is not strictly necessary to have postgraduate qualifications. This offers the opportunity for permanent employment in a specific field, early in your career, without the stress of chasing apprenticeships, moving frequently, and taking postgraduate exams. SDs can become highly experienced clinicians and often work with a high degree of autonomy and are therefore very valuable members of their team.

SDs, if they choose to do so, can develop their role clinically, in management or teaching, helping to maintain a high level of job satisfaction, personal performance and respect from our colleagues. A set of additional safeguards and provisions to support the health and well-being of SAS physicians.
